Dear Valued Customers,

    In our continuing effort to better serve you, we are excited to announce our new and improved order form.  Our new form uses the latest technology to allow you to download our order form, fill it in completely on your computer (by either clicking in the checkboxes or placing your cursor over the line on which you want to enter information and clicking), and submit it to us via email.  Or if you wish, you can simply download the new form, print it out, and fax it in or bring it by just like always.

    One thing we ask, when you submit your form electronically, please type your name and the date on the signature line as your “electronic signature”.

    Starting today, we will no longer be accepting orders from the old online form submission, as it is outdated and somewhat more difficult to use.
